An example of adenoviral gene therapy in the human diabetic organ-cultured corneas is presented towards the normalization of delayed wound healing and markedly reduced epithelial stem cell marker expression in these corneas. It also describes the optimization of this process in stem cell-enriched limbal epithelial cultures.
Das Ziel dieses Protokolls ist molekularen Veränderungen in humanen diabetischen Cornea zu beschreiben und zu zeigen, wie sie durch adenovirale Gentherapie in organ kultivierten Hornhaut gelindert werden. Die diabetische Hornhauterkrankung ist eine Komplikation des Diabetes mit häufigen Störungen der Hornhautnerven und epitheliale Wundheilung. Wir haben auch signifikant verändert Expression mehrerer putative epitheliale Stammzellmarkern in humanen diabetischen Cornea dokumentiert. Um diese Änderungen, adenovirale Gentherapie lindern wurde erfolgreich die Hochregulation von c-met proto-Onkogen-Expression durchgeführt und / oder das Herunterregulieren von Proteinasen Matrix-Metalloproteinase-10 (MMP-10) und Cathepsin F. beschleunigte Heilung dieser Therapie bei diabetischen Cornea gewickelt selbst dann, wenn nur der Limbus-Stammzellen-Kompartiment wurde transduziert. Die besten Ergebnisse wurden mit einer kombinierten Behandlung erhalten. Für mögliche Patienten Transplantation von normalisierten Stammzellen ist ein Beispiel auch der optimizati vorgestelltauf der Gen-Transduktion in Stammzell-angereichertem Kulturen unter Verwendung von polykationischen Verstärker. Dieser Ansatz kann nützlich sein, nicht nur für die ausgewählten Gene sondern auch für die anderen Mediatoren von cornealen epithelialen Wundheilung und Zellfunktion stammen.
Die diabetische Hornhauterkrankung führt vor allem zu degenerativen epithelialen (Keratitis) und Nerven (Neuropathie) ändert. Es wird oft von den Anomalien der epithelialen Wundheilung und Hornhautnerven Reduktion 1-4 manifestiert. Schätzungsweise 60-70% Diabetiker haben verschiedene Hornhautprobleme 1,3. Unsere Studien haben mehrere Markerproteine mit veränderten Expression in humanen diabetischen Augenhornhaut einschließlich der Herunterregulation von c-met-Protoonkogen (Hepatozyten – Wachstumsfaktor – Rezeptor) und die Aufregulation der Matrix – Metalloproteinase-10 (MMP-10) und Cathepsin F 5, identifiziert 6. wir haben auch deutlich verringerte Expression von mehreren putative epitheliale Stammzellmarkern in den humanen diabetischen Cornea dokumentiert.
In den früheren Studien haben wir ein adenovirales basierten Gentherapie zur Normalisierung der Pegel von Diabetes-altered Markern unter Verwendung von humanen diabetische korneale Organkultursystem entwickelt, das langsame Wundheilung zeigt, diabetischeMarker Änderungen und die Expression Zellmarker Reduktion Stammzellen ähnlich wie die ex vivo Cornea 7,8. Diese Persistenz der Änderungen angezeigt 9 aufgrund der Existenz von epigenetischen metabolischen Speicher sein. Dieses Kultursystem wurde weiter verwendet für die Gentherapie. Die Ziele für diese Therapie wurden entweder mit reduzierter Expression bei diabetischen Cornea (c-met proto-Onkogen) oder erhöhte Expression (MMP-10 und Cathepsin F) aus Markern ausgewählt.
Das Adenovirus (AV) Therapie wurde in den gesamten Orgel kultivierten Cornea oder der korneoskleralen peripheren Limbus Raum nur verwendet. Dieses Fach beherbergt Stammzellen epithelialen, die das Hornhautepithel erneuern und sich aktiv an der Wunde 4,10-15 Heilung teilnehmen. Hier Protokolle werden für normalen und diabetischen menschlichen Cornea-Organkultur, epitheliale Wundheilung, Isolierung und Charakterisierung von Stammzellen angereicherte Limbus Zellkulturen und adenoviralen Zelle und korneale Transduktion vorgesehen. UnserErgebnisse zeigen die Machbarkeit dieser Therapie für Markerexpression zu normalisieren und bei diabetischen Cornea für mögliche künftige Transplantation Wundheilung. Sie legen auch nahe , dass die Kombinationstherapie die wirksamste Weg ist , 16-18 normalen Markierungsmuster und Epithelheilung in der diabetischen Cornea wiederherzustellen.
Die Hornhaut wird aufgrund seiner Oberflächenstelle ein ideales Gewebe für die Gentherapie, wo die Genübertragung, sowie die Bewertung der Wirksamkeit und Nebenwirkungen, einfach sind. Allerdings ist eine klinische Umsetzung dieser leistungsstarken Ansatz immer noch langsam aufgrund knappen Informationen über die genetischen Ursachen der Erkrankungen der Hornhaut und der Gentherapie Targets 24.
