Assessing a patient's airway is crucial in health evaluations and involves checking for obstructions like vomit or foreign objects.

Assess airway patency in conscious patients by asking questions and receiving a verbal response. A normal verbal response indicates a clear airway and adequate breathing, ensuring proper brain perfusion.

Assess breath sounds and note adventitious sounds like wheezing or stridor, and observe if they occur during inspiration, expiration, or rest.

Next, assess the skin color, a crucial indicator of respiratory health.

Cyanosis, a bluish skin and mucus membrane discoloration, occurs when arterial blood oxygen saturation falls below 85%.

Cyanosis is a late sign of respiratory deterioration, often hard to detect under artificial lighting.

In addition, assess the use of accessory muscles for breathing. Normal breathing primarily involves the diaphragm and intercostal muscles.

Observe the patient's neck during inspiration for contraction of the sternocleidomastoid or other accessory muscles, indicating respiratory distress.

Also, observe the shape and expansion of the chest. Increased anteroposterior chest wall diameter and asymmetrical chest wall expansion may indicate severe airflow limitation.

Assessment of Airway

It involves checking for obstructions that could impede breathing, caused by factors such as vomit, foreign objects, or the patient's tongue. A quick method to assess airway patency in conscious patients is to ask them to speak; a normal verbal response confirms that the airway is clear and the patient is adequately breathing and perfusing their brain.

Skin Color

Skin color is a vital indicator of respiratory health. Cyanosis, characterized by a bluish discoloration on the skin and mucous membranes, signals high levels of unsaturated hemoglobin in the blood, typically detectable when arterial blood oxygen saturation falls below 85%. Cyanosis is a late sign of respiratory deterioration and can be difficult to discern, especially under artificial lighting. It is divided into two types:

  • • Central Cyanosis: Affects the lips and oral mucosa, indicating potential cardiorespiratory insufficiency.
  • • Peripheral Cyanosis: Appears in the skin and nail beds, often signifying poor peripheral circulation when observed in isolation. Anemic patients may not display cyanotic symptoms due to insufficient hemoglobin, and pale skin may indicate anemia or shock.

Use of Accessory Muscles

Observing the use of accessory muscles provides insight into respiratory effort. In normal breathing, the diaphragm and intercostal muscles should suffice. The use of neck, chest, or abdominal muscles suggests respiratory distress. Monitoring the rhythm, rate, and depth of respiration, along with chest shape and expansion, also offers valuable information about a patient's respiratory health.
