Physical assessment is another data collection method, and it determines a patientƉs state of health by reviewing their body systems. This approach to patient examination is systematic and focuses on each region's underlying anatomic structures, their functions, and possible abnormalities. The procedures and steps of physical examinations require verbal consent from the patient. The physical assessment methods follow a four-step IAPP procedure – namely, inspection, auscultation, percussion, and palpation. Inspection is the visual examination of the patient. Auscultation involves listening to various organ sounds using a stethoscope. Percussion is carried out by tapping over internal organs, joints, and muscles and assessing the density of the underlying tissues for fluid, air, or solid fill. During palpation, minimal pressure is exerted on various organs to identify their size, contour, and tenderness.
