El desarrollo y la prueba de dispositivos endovasculares para el tratamiento del aneurisma intracraneal siguen siendo de gran importancia. La mayoría de los modelos de aneurisma utilizados hoy en día pasan por alto las características importantes de una pared arterial degenerada o la hemodinámica de una verdadera bifurcación. Por lo tanto, nuestro objetivo fue diseñar un nuevo modelo de bifurcación de bolsas arteriales en conejos.
El tratamiento endovascular para los aneurismas intracraneales ganó importancia en las últimas décadas, por lo que existe una mayor necesidad de probar dispositivos endovasculares. Los modelos animales que respetan las condiciones reológicas, hemodinámicas y de la pared del aneurisma están altamente justificados. Por lo tanto, el objetivo del presente estudio fue diseñar una nueva técnica quirúrgica estandarizada y reproducible para crear aneurismas de bifurcación de bolsa arterial autóloga con condiciones de pared no modificadas y modificadas en conejos.
Los aneurismas de bifurcación se crearon por anastomosis de extremo a lado de la arteria carótida común derecha en la izquierda, ambas sirviendo como arterias madre para la bolsa arterial, que fue cosida microquirúrgicamente. Se tomaron injertos de la arteria carótida común proximal derecha, ya sea para el grupo control (n = 7, reimplantación autóloga inmediata) o modificado (n = 7, incubado con 100 unidades internacionales de elastasa durante 20 minutos antes de la reimplantación autóloga). La permeabilidad de la bolsa y la arteria madre se controlaron mediante angiografía de fluorescencia inmediatamente después de la creación. En el seguimiento (28 días), todos los conejos se sometieron a angiografía por resonancia magnética mejorada con contraste y angiografía por fluorescencia seguida de recolección de aneurismas, evaluación macroscópica e histológica.
Un total de 16 hembras de conejos blancos de Nueva Zelanda fueron operados. Dos animales murieron prematuramente. En el seguimiento, el 85,72% de todos los aneurismas seguían siendo patentados. Ambos grupos revelaron un aumento en el tamaño del aneurisma con el tiempo; esto fue más pronunciado en el grupo control (6,48 ± 1,81 mm3 en el momento de la creación frente a 19,85 ± 6,40 mm3 en el seguimiento, p = 0,037) que en el grupo modificado (8,03 ± 1,08 mm3 en el momento de la creación frente a 20,29 ± 6,16 mm3 en el seguimiento, p = 0,054).
Nuestros hallazgos demuestran la idoneidad de este nuevo modelo de conejo que permite la creación de aneurismas de bifurcación con diferentes condiciones de pared en un enfoque microquirúrgico. Dada la excelente permeabilidad a largo plazo y la propiedad del crecimiento del aneurisma a lo largo del tiempo, este modelo puede servir como una herramienta importante para la evaluación preclínica de nuevas terapias endovasculares.
La hemorragia subaracnoidea resultante de la ruptura del aneurisma intracraneal (IA) puede controlarse eficazmente mediante técnicas de oclusión endovascular o microquirúrgica 1,2,3,4. Diferentes terapias endovasculares, para superar la principal limitación de la recurrencia de la IA después del enrollamiento, ganaron importancia en las últimas décadas generando una mayor necesidad de probar dispositivos endovasculares. Para probar estos nuevos enfoques de tratamiento, los modelos animales apropiados que respetan las propiedades reológicas, la hemodinámica y las condiciones de la pared del aneurisma están altamente justificados 5,6,7. En este contexto, tanto los estudios clínicos como preclínicos ya han revelado el importante papel de las afecciones de la pared del aneurisma con respecto a la ruptura y recurrencia del aneurisma después de la oclusión, centrándose especialmente en la pérdida de células murales 7,8,9.
Hasta ahora, los aneurismas experimentales en conejos se han creado con mayor frecuencia ya sea por tocones de arteria carótida común (CCA) incubados con elastasa o bolsas venosas suturadas en una bifurcación artificial de CCA. 10,11,12,13,14,15,16 Así, nunca se ha descrito un verdadero modelo de bifurcación de bolsa arterial.
El objetivo de este estudio fue diseñar una técnica segura, rápida y estandarizada para la creación microquirúrgica de aneurismas de bifurcación con diferentes condiciones de pared en un modelo de conejo (Figura 1). Esto se logró suturando bolsas arteriales no modificadas y modificadas en una bifurcación artificial creada de ambas CCA.
Nuestro estudio demuestra la viabilidad de crear un verdadero modelo de aneurisma de bifurcación con diferentes condiciones de pared en conejos. En general, se incluyeron en el estudio 14 conejos blancos hembra de Nueva Zelanda con un peso medio de 3,7 ± 0,09 kg y una edad media de 112 ± 3 días. El 85,72% de todos los aneurismas permanecieron patentes durante un seguimiento a los 28 días. Dos animales murieron prematuramente (12,5% de mortalidad).
